Application for Training Class

Signup for class is a three part process. All three steps must be completed for your application to be processed and a class assignment made.  Each step can be completed at different times by  returning to this page.

  1. Complete the Class Application Form
  2. Send a copy of your dog’s vaccination record.
  3. Payment.

Send a copy of your dogs vaccination records

Current rabies vaccination, administered by a licensed veterinarian is required. Check with your vet about other recommended vaccinations for dogs in contact with other dogs.  You may upload most common file types, including .jpeg and .pdf.
